Enrollment is based on availability.

 

Now enrolling for January and September.

Before children are admitted, parents are required to fill out an enrollment packet containing the following forms:

​

• Child’s Enrollment Form

• Developmental History and Background Information

• First Aid and Emergency Medical Care Consent Form

• Health Care Provider Examination (copy of most recent physical to be updated yearly)

• Copy of child’s Immunization Record

• Copy of child’s lead test
